It is a big step when you finally decide to ask for help. When starting on the opiate agonist program at one of our Hope Health Clinics locations we try to make the process as easy as possible. All you need to bring with you is your
- Health card or photo identification
- List of current medication
- Names of any specialist or other physicians that you see
- Any other information you would like to share.
You will be asked to fill out paperwork prior to meeting our physicians who will tailor a treatment plan according to your unique situation.
Majority of our patients will start treatment on the same day as their appointment.
